CHANGE OF REGISTERED NAME

Medical practitioners who change their name are encouraged to register the change with the Medical Board as this assists Board staff with enquiries from the general public, and ensures that the Certificate of Registration is accurate. If a medical practitioner wishes to change his or her registered name, the Board requires a certified copy of the marriage certificate or a statutory declaration evidencing the change of name. To register a change of name, please use the Application for Change of Name � FORM L (available in Registration Forms section) and pay the applicable fee of $30.

CHANGE OF ADDRESS

The Medical Board Register has provision for both a practice and/or postal address for practitioners. Section 58 of the Medical Practitioners Act 2008 requires medical practitioners to provide written advice to the Board of any change to their address that is recorded in the register within 30 days of a change of address. Please use the Notification of Change of Address � FORM Q (available in Registration Forms section).

Failure to notify such change may result in the medical practitioner not receiving the annual registration reminder notice and therefore the practitioner's name may be erased from the Register for non payment of fees. A fine of $1000 applies if the Board is not notified of a change of address.

CERTIFICATE OF REGISTRATION STATUS
(previously known as Certificate of Good Standing)

Requests for Certificates of Registration Status must be lodged in writing with the Board. Certificates will be forwarded directly to the nominated Medical Board, Council or College on the practitioner's behalf. A $50 fee is applicable. Certificates of Registration Status are valid for a period of three months. Please use the Application for a Certificate of Registration Status � FORM K (available in Registration Forms section).
